Dermatofibrosarcoma protuberans is a rare, slowly growing malignancy that arises from the dermis. Clinically, the tumor features a high recurrence rate and a low distant metastasis rate. There are few cases of extensive facial dermatofibrosarcoma protuberans that have been reported and indicated the necessity of a multispecialty approach to managing this rare disease. We reported a 28-year-old woman with a huge facial dermatofibrosarcoma protuberans. This tumor located in the left frontal-facial region and caused left blindness. She underwent en bloc resection and staged reconstructive operations. The tumor was removed completely under the periosteum plane. We also introduced a method to decrease bleeding and benefit reconstructive surgery using a Rummel tourniquet for temporary occlusion of the external carotid artery. Our management was successful after temporary occlusion of the external carotid artery, adequate surgical margin, and delicate subperiosteal dissection of the facial bone and the patient was satisfied with her appearance and functional outcome.(J Taiwan Otolaryngol Head Neck Surg 2020; 55:90-94)
