A 75-year-old male with type 2 diabetes mellitus suffered from 5-month of severe pharyngodynia. Initial laryngoscopic examination revealed laryngeal ulcerations. After biopsies of the posterior pharyngeal wall and of skin blisters that were incidentally found on the thighs, the patient was diagnosed with bullous pemphigoid (BP). The laryngeal ulcerations gradually resolved after two weeks of steroid therapy. This case highlights the importance of recognizing and managing laryngeal involvement in BP.
