Bronchogenic cysts are rare congenital malformations that originate from aberrant budding of the tracheobronchial tree during its development. They are commonly located in the mediastinum and lung parenchyma and infrequently noted in the cervical region. Because there are no distinctive clinical presentations or radiological findings for such cysts, accurate preoperative diagnosis is difficult. Complete surgical resection with histopathological confirmation is required for both treatment and definite diagnosis. We present a 35-year-old man of a bronchogenic cyst located in the anterior neck since childhood. T2-weighted magnetic resonance imaging showed a characteristic high-intensity cyst located between the right thyroid gland and sternocleidomastoid muscle. The mass was surgically removed and confirmed to be a bronchogenic cyst. He had no complications or recurrence postoperatively. The present study describes this rare disease in an unusual location, mimicking a branchial cleft cyst. We report the case for its typical radiological features and pathologic findings for cervical bronchogenic cyst.(J Taiwan Otolaryngol Head Neck Surg 2020; 55:105-109)
