Ramsay Hunt syndrome (RHS), caused by the reactivation of the varicella-zoster virus in the geniculate ganglion, typically presents with peripheral facial nerve palsy (FNP), auricular vesicular eruptions, and otalgia. The involvement of additional cranial nerves, such as the glossopharyngeal (IX) and vagus (X) nerves, is rare but has been documented in the literature. Herein, we report the case of a 47-year-old woman presented with right aural fullness and otalgia, right-sided facial palsy, and mild dysphagia with choking. We further offer a brief summary of the literature related to its epidemiology, diagnosis, treatment, and prognosis. (J Taiwan Otolaryngol Head Neck Surg 2026; 60:47-52)
