Transoral robotic surgery (TORS) is an alternative treatment for obstructive sleep apnea (OSA) patients who cannot tolerate continuous positive airway pressure well. We reported an OSA case with impending extubation failure event after TORS. We found that an omega-shaped epiglottis considerably contributed to this event, which was not highlighted in previous studies. (J Taiwan Otolaryngol Head Neck Surg 2022; 57:287-292)
