Hypopharyngeal liposarcoma is an extremely rare type of soft tissue sarcoma. We report a case of a 60-year-old man presenting with progressive dysphagia and a foreign body sensation in the throat, which were notably aggravated by changes in head position a symptom not previously described in similar cases. Despite thorough diagnostic imaging and endoscopy prior to further treatment, a definitive diagnosis remained elusive, with only a tumor-like lesion observed in the right hypopharynx, highlighting the diagnostic challenges often associated with this condition. The patient subsequently underwent transoral laser microsurgery, and histopathological analysis confirmed a well-differentiated liposarcoma characterized by MDM2 and CDK4 oncogene amplification.Long-term follow-up is essential due to the risk of local recurrence, despite the tumor's low metastatic potential. This case adds to the limited literature on hypopharyngeal liposarcomas and highlights the need for a multimodal approach in diagnosis and treatment.
