The parapharyngeal space is shaped like an inverted pyramid. In this space, benign tumors account for approximately 80% of all tumors that pleomorphic adenoma is the most frequently encountered. It can develop de novo or may arise from the deep lobe of the parotid with extension through the stylomandibular tunnel into the parapharyngeal space. Early symptoms are non-specific, usually not significant. We presented a case of right primary parapharyngeal pleomorphic adenoma having right facial palsy as the initial symptom. Tumor excision was done through the transcervical-submandibular approach. There was a dramatic recovery from facial palsy in the postoperative recovery room.
